I understand that this document is a legally binding contract.
The Sanctuary reserves the right to ownership of the animal should the contract be breached. I am happy for a member of the Sanctuary to visit.
I promise that Sammy will be kept in the house, as a member of my family.
He will be properly fed and exercised, and will receive Veterinary treatment whenever necessary.
I understand that Sammy cannot be sold, given, or passed to any other person or animal group, without the Sanctuary’s prior consent. Nor can he be euthanized without prior consultation with the Sanctuary - except in a medical emergency to ease suffering. If at any time throughout his lifetime I am unable to keep him for whatever reason, he will be returned to the Sanctuary.
There is no guarantee that the dog is, or will be, free of disease or behavioural disorders. We make every effort to confirm that a dog is healthy and socially acceptable before placing it. We cannot, however, exclude the possibility of a lurking disorder in a particular animal.
I understand that the Sanctuary have passed on, in full, all the information they have about the dog concerned, and accept that in signing this form, I assume full legal responsibility,
and liabilities for the above mentioned dog.
Lizzies’ Barn Sanctuary is a non profit making rescue and relies on donations, to continue our rescue work. It costs the Sanctuary an average of £250 per dog, from rescue through to re-homing. To allow us to continue our work a non-refundable donation of a minimum £160 payable to the Sanctuary, is expected at the time of adoption

Sammy came from a family home: Mum, Dad & a little girl of 3. He was given up due to redundancy causing his family to lose their home.
He has been a much loved family member since he was a puppy & his family are devastated to lose him.
Sammy loves people & is very friendly & affectionate.
He is good with the majority of other dogs, but has clashed with the occasional uncastrated male on his home territory - he has only recently been castrated & is expected to mellow.
Sammy does not like cats.
He is house trained & well behaved indoors. Sammy loves to play with toys, particularly soft toys & ball games.
He can be left for reasonable periods.
Sammy is happy in the car & travels well.
